This method was plagued with multiple challenges and gaps —last minute shift changes, clinician burnout, and inconsistent coverage. The client needed a scalable and intelligent scheduling solution that would align with their staffing with patient demands, clinician availability and institutional rules. OSP proposed an AI-augmented scheduling system that uses Linear Programming with agentic AI to intelligently assign shifts and eliminate inefficiencies. OSP designed and developed a modular healthcare-native scheduling platform, which integrated seamlessly with the client’s existing infrastructure. Integrating Epic EHR, UKG workforce management system and Workday via FHIR APIs, we enabled real time flow of critical scheduling data. We included these key integrations — demand forecasts from Epic for 90 day predictive patient volume by unit, clinician supply from Workday/UKG to check role-based availability, credibility and shift history, staff preferences via internal portals that is captured through responsive agent interface, and scheduling rules from HR and compliance teams that includes policies related to fatigue, labor laws and union agreements.
The core of our solution is Linear Programming (LP) engine, which automated the shift allocation while adhering to a multitude of hard and soft constraints. We included features like constraint-sensitive scheduling, preference based matching and conflict resolution. Constraint-sensitive scheduling took into account staff’s rest periods, maximum weekly hours, and even required skills and legal mandates. The preference-based matching considered clinicians’ preferred time slots and off-days. Conflict resolution automatically reduces manual overrides and shift swaps. This programming complemented by AI agents helped eliminate the hassle of manual planning, making scheduling more adaptable and flexible.
We prioritized security and compliance from day one. The system encrypts all data using AES-256 and TLS 1.2+, ensuring full HIPAA compliance. Role-Based Access Controls (RBAC) restrict access to sensitive scheduling data, keeping it secure and role-specific. Immutable audit logs track every schedule update, swap, or preference change to support audits and internal governance. FHIR-native integration enforces minimal data usage, respecting the need-to-know principle in clinical workflows. These safeguards allowed the client’s legal and IT teams to confidently adopt and scale the platform across their organization without compromising regulatory or data security standards.
Our solution empowered managers to respond instantly to staffing changes. The Simulation Bot helped the hea test what-if scenarios and adjust schedules on the fly. The NLP Explainer answered queries like Why isn’t Dr. Rao working Friday? with clear reasoning based on rules or constraints. The Swap Validator approved or rejected shift changes in real time, maintaining balance and compliance. Clinicians used the Preference Collector to submit availability or preferences directly through mobile and web portals. Through these tools, we aimed to transform scheduling from a static task to a dynamic, daily process that supports both flexibility and operational efficiency.
